Please open and review all of the documents below.  After reviewing the policies reply ‘YES’ to your confirmation text message, or click ‘Confirm Now’ on your confirmation email, to indicate that you agree with our policies and would like to keep your appointment.   Your ‘YES’/‘Confirm Now’ response replaces all in-office paperwork.
